printing single patient ledger-include doctor's name

we want to be able to add the providers name (not provider id) in the printout of single patient ledger and also the family ledger when we decide to print

When entering a patient insurance payment, there should be a place where you can make a note as to the patient's co-insurance amnt due.

This would be useful for anyone looking at a particular claim, to see what the EOB states the patient's co-insurance is regarding that particular DOS.
