Add rendering provider on "Procedures Not Attached To Insurance Claims" report.

To verify that procedures are billed under the correct provider of service.

Adding NDC#

For offices that are submitting medical claims, we need to be able to input any NDC#s (if needed) to the claim form; currently, we have to write this information manually.
