We have contracted rates that are posted to patient ledger but the claims need to send to insurance with full office fees. There is a 2024 claim form but it sends the posted fees not UCR for the office. The last claim form that has this is 2012F, the Medicare plans are starting to deny claims for the incorrect claim form and are asking for the 2024 form with full fees
