Within the patient chart, we would like the ability to electronically fill out a consent form on the PC so that when a patient signs the form on a laptop with touchscreen for example, it can eb printed with the filled in information. For example, the consent form for Crowns has blank lines and checkboxes to fill out information. The box should be editable to fill out that information so when signing on the PC, it can be printed with all the necessary information.
