Some of the filling locations are not where cavities occur. Lingual fills on upper incisors occur on the lingual pit, not midway up next to the incisal edge. Class V cavities by definition, are always along the gum line, even when they extend into the mesial or distal, currently, a MF5 is graphed like it's an anterior MF filling (ignoring the class V part).
