Very displeased with the newly added Dentrix Patient Forms. There should be a way for the Practice to see how many times a form has been sent to a patient and when it was sent. We should be able to see a history of sent and submitted forms for every patient. The KIOSK is extremely limited. The Practice should be able to send any and all kinds of forms needed. Such as New Patient forms, Medical History, Consent Forms, Dental/Surgical Instructions, etc. As of now, the KIOSK can only be used for first time New Patient Forms. It is also an inconvenience waiting on a verification code in order for the patient to fill out forms. We have had instances where the patient did not have their phone with them, so we are still having to do paper forms. It shouldn't require a verification code, it should be the patient's information (i.e. date of birth and last name).
