need to add tooth # and surface. should not have to do.

Have you tried updating the procedure code information?
Office Manager > Maintenance > Practice Setup > Procedure Code Setup - when the window opens, find the D4346 and click "edit" - when the box pops up, in the lower left corner, make sure the "treatment area" drop down box says "mouth" and that should do it.
