Update fees on scheduled patients and pte

After updating a fee schedule, the fees for the patients already scheduled do not get updated, previous fees are set complete.

    Go into any chart, click "treatment manager" (green chair, no money sign) - when the window pops up, click on the money sign with the arrow under it "update treatment plan fees" and a window will open - select update fees for "all patients" in drop down and click "update."

    Takes a few minutes but every chart gets updated at once.
