I am getting rejections of codes D4241; D4261; D4342 electronically filing the claim to insurance companies. I need these codes to have quadrant, and tooth numbers BOTH in order for the error to not incur. for example; I can go to Delta AZ and enter a claim for D4241 and type UR, and #3 in both fields. but if I do it electronically the claim is rejected asking for Quadrant!? so aggravating because the claim does not go thru and comes back to me as a delay in pmnt. please fix this. I don't want to say UR quadrant ON MY END when the code is set up for 1,2,or 3 teeth only! I need to tell our providers what tooth they are to work on -it's not a quadrant code!
