Add a box in the forms to check when a paper scanned document is in (ex: Medical History)

It would be beneficial to add a box to the forms to check mark a scanned in paper document and add the date it was received so that the date will show that the document has been received. (We have some people who do not want to complete electronic forms). Then when you go in to manually change the date due, you cannot just type a date 5 years into the future and it does not matter as it will continually say that the document has not been received.
