When we click along the note window pop ups- once the decision of "yes" "no" "reviewed", etc, right now when things are chosen and submitted to the notes it does not tell us what the yes, no, reviewed is for, it would be nice to see the title of the reason so in the clinical notes it would say "medical history- reviewed", "premedication taken- yes"
