We are a pediatric, orthodontic and adult dental practice that offers (5) very different patient forms. We instruct the patient which of the (5) forms pertain to them for that visit- not ALL pertain. However, the message on the form page states "All forms are required by your provider" This is simply NOT TRUE- It is contradictory to what we are telling our patients.
That we can not easily change this message is very frustrating
