I want to be able to send a link for a specific form/questionnaire to a specific patient. What is the purpose of being able to make a questionnaire if I cannot send it to a specific patient? I also want to be able to send the med hx update form in an individual freeform text and NOT through the "check-in" optin under Today's Follow Up Actions. Dentrix is so limited with the forms section.

YES! We should be able to have individual links for kid and adult patients to make online forms specific to each patient.
Agree! this has been a significant issue on our Pediatric practice. With multiple children families paperwork is often missed for only one child.
Very much agree we would like each form to have its own link we can send. Why we can't send whatever form we want to whatever patient we want whenever we want is crazy.
