The pharmacy, physician, and emergency contact info should be integrated from the Health history form instead of having to enter it manually for each patient. This will save a lot of time and duplication of information

This is a serious issue that needs to be addressed. Patients forms should not be this challenging to update. We do not want this information to get misplaced. Not only does the emergency contact field need to be entered but also the physician and the pharmacy details. This was brough up in 2021 and its 2023, this needs to be updated accordingly.

yes!!!!!! This is 100% more important information than where a patient is employed. I cannot believe this is not an option.
