Right now if you add a condition, allergy you can add to questionnaire ( in error for us) , thus changing the form again. Many of us use this as our chart and have copies of it on other platforms. so when it is consistently changed things are different on the other sites . Would like to be able to fix this form so that only administration can amend the questionnaires questions .
