In the treatment case set up, create a case note. Here is how we have ours set up and I have it called Tx acknowledgement:
I have been informed of the dental diagnosis and the recommended dental treatment as listed on the attachment to this acknowledgment. I am aware of the consequences of non-treatment.
TREATMENT ON A MINOR (CHILDREN UNDER THE AGE OF 18) REQUIRE THE PRESENCE OF A PARENT OR LEGAL GUARDIAN.
