I have added commonly found/reported conditions and an extensive list of medications from my patients' medical histories over the past year and am wanting to develop a digital questionaire that new patients and returning patients could use. I would like to offer in-office on a tablet that links to Dentrix as well as linking to my website. Printable option would also be good for those who prefer paper. I would want a way to have patients upload a medication list, MD list, emergency contact, also.
