If I insert the PI -Health History checkboxes, it has all medications, allergies and medical conditions listed together in alphabetical order. These need to be separated just like they are in the Health History module so the info will upload and the patient isn't confused when filling out the questionnaire.

also include a PI for Emergency contact , Pharmacy and medical doctor
