PLEASE PLEASE PLEASE can we revise the check box portion of the health history, patients find it VERY confusing and the conditions, allergies and medications need to all be separated into different categories. I would LOVE for someone in charge of this to call me so we can discuss ideas on how to revise this so it will be more meaningful for the office and easier to understand for the patient.

AMEN!!! Have spent an entire Friday trying to deal with this. Super frustrating. Could be so easy. Also, disease category should be able to be edited. What one dentist/ specialist sees as important disease to be aware of........ another may not. Some see kids, some geriatrics, some do more surgical some not.
