Frequently, patient calls up for a limited exam or even recall appointment it does not display in the chart. If that happens, frequently in order to get the consent for the procedures, it has to be reentered or it will not show up in consent form. Also duplicate codes are a frequent nuisance, because it does not display in the chart what is in the appointment book.
