When you populate a questionnaire with the prebuilt "Patient Information" fields, it can fit multiple questions on one line (ex. Last, First, Middle Initial are all on the same line). When creating your own questions on the questionnaire, it will only allow you to put one question per line, even if it is a short text field that could fit 3-4 per line. It would condense a lot of our forms and make it more visually appealing, as well as more streamlined and comprehensive. Thank you for your time, have a good day.
