There needs to be better tracking for sent forms to patients.
1 -- we should get alerts or notifications whether in Dentrix or via email that a patient completed forms including forms sent on demand
2 -- the forms portal should have a summary page showing all forms sent (both automated and on demand) and the status (i.e. completed, not started, started, etc...).

Yes, we need to have a date and time stamp for when forms were sent ... multiple times if same form was sent multiple times.
Currently, the portal only shows a summary of forms that were completed. This is not really helpful since we can see those in the patient chart. We need the ability to see who was sent forms but didnt complete.
