1. Can we add a line in Patient Registration form about Preferred Pharmacy name that Patient would like to use.
2. HIPPA Consent Form should have several fields to type in people's names that "patient gives permission to our office to disclose my health, treatment and diagnostic records (this includes all past, present and future period of health care information) which will include relation to patient.
