In the new Dentrix forms it allow for yes/ no answers that go directly into the Health History under conditions or allergies. Some of these responses require additional follow up. You can create a separate question for the form, but then it will not link with the Health History. Right now the patient can only answer yes or no, with no follow up. Please see the attached photo of what I envision working best.
