misc. note panel in chart

I think a miscellaneous note panel in chart for info such as; gags easily, prefers no cavitron, cannot recline, has a parastesia, etc. We should not have to"look" for notes in other modules.

Chart refresh of Smart Image

Thumbnails of Smart Image do not refresh when Chart is refreshed. You physically have to close the Chart, then re-open the Chart for the thumbnail changes to take effect

Take off ability to remove all appointments from a select date to pinboard list
