List teeth differently for code D4342 on claims and pre-estimates

Some insurance companies are kicking back eclaims and epre-estimates because the teeth are listed like this (2-4) instead of like this (2,3,4). This means we have to resubmit the claims by mail which takes forever to be received and processed.
