Customize form responses format for printing

It would be useful for practices in Canada to change the format of the form responses prior to printing them from Dentrix Hub. The current format is a bit messy and unorganized (see screenshot attached).

Consent for electronic statement via text on Forms

Can you give an option for patients to approve or deny receiving electronic statements with a link to pay their bill that automatically updates the family file, please?

TO BE ABLE TO RETRIEVE A DELETED QUESTIONAIRE

After Importing the patient's online questionaire, it was deleted accidently within the questionaire tab. We should be able to retrieve deleted questionaires.

Remove item description on Dextrix Hub forms

There was an issue with Dentrix Hub forms were the description of the item (textline, radio button, etc) is showing up under the title and text description. Doesn't look polished when we are showing this to patients.
