Add the ability to read the ledger notes to people without granting them adjustment permissions.

There is no ability for people without adjustment rights to read the notes being left on the ledger. This inability to read notes renders them useless.

Add ability to use full description of service on billing statements

The user could select if the billing would use a full description (patient friendly description) that is associated with an ADA code. Expanding the characters in the description name would be beneficial
