Add status of Deceased to Patient Stuts

It would be very helpful if we had more status choices other than Patient, Non-Patient, Inactive, and Archived. When a patient passes, this would be a much easier way to show a patient Deceased.

Edit phone numbers on continuing care report

Many people do not have home #'s. Most use mobiles #s and calls are made during the day when patients are not home. Please allow to modify or change the format the mobile instead.
