Removing the excessive space between state and the zip code

When we print out statement , we are showing a big space between the state and zip code, the mail is being returned due to the big space.

Add status of Deceased to Patient Stuts

It would be very helpful if we had more status choices other than Patient, Non-Patient, Inactive, and Archived. When a patient passes, this would be a much easier way to show a patient Deceased.
