CDA form-adding info

If we could fill out the CDA form in Dentrix and not have to manually fill it out, it would look more professional. This would be beneficial for 3rd insurance plans.

Keep original date xray was taken when importing to claims

Add claim total to EOB response

Add total claim amount to the bottom of EOB response. Allows for easier calculations for patient portions and it is easier for patient's to understand.

Range of teeth numbers for SRP

When doing localized SRP, insurance companies request the tooth number(s), the ability to add the teeth would make the claims process so much better. i.e. #11, #12 or #13, #15
