Reports for insurance downgrades/denials

We have a lot of adult prophys that get downgraded to child prophys. I would like to be able to run a report so I can appeal prior claims that were written off prior to me starting at this office.

Blank ADA form

It would so extremely useful if you would allow the Blank ADA form to not only print out but allow to become a PDF so we could fill it out. My office doesn't have a typewriter so I would have to write it all in and my handwriting is not very good.
