Add option to show appointment time when viewing/printing schedule

It is hard to tell what the actual appointment time is for all columns but the first and last. It would be great to have the option to view/print the APPOINTMENT TIME along with things like Name, Age, Appt Reason, Appt Type, etc.

Assigning Procedure code exam to a specific provider.

It would be great if certain codes mainly all the exam codes could be only assigned to the main provider so when the hygienist is closing out her appointment the exam goes to the dr., reference number for the call 05074332
