Decimal needed for deductible coverage table

A small handful of carriers have $xx.xx deductible, instead of the usual full dollar amount deductible (ex: Delta of WA for particular groups have a $37.50 deductible).

pre authorization requirements

Longer names for Fee Schedules

We need to be able to add more than just 7 characters to the fee schedule names. We have several different insurances with different plans and we need to be able to tell the difference between the plans when verifying and connecting the insurances...
